Background: Ethical competencies of managers are one of the most important components of improving the quality of governance, administrative health, and organizational efficiency; especially in the healthcare sector, where the breadth of duties, volume of human resources, and complexity of services require a specific framework of ethical competencies. The aim of the present study is to identify the components of ethical competency of healthcare managers. Method: This qualitative review study was conducted with a meta-synthesis approach. The research community included domestic and foreign studies conducted between 2000 and 2025, and 21 eligible studies were selected. The coding method was used to analyze the data. Results: The results of the analysis showed that the ethical competencies of managers are organized into three categories: behavioral, contextual, and structural, and have 35 subcomponents. The most frequent were behavioral and contextual competencies. Conclusion: The ethical competency of managers is not limited to individual skills and requires structural mechanisms, supportive policies, and organizational culture. Strengthening these competencies can help promote public trust, improve the quality of health services, and make management structures in the health and treatment sector more efficient.
